ACCIDENT / INJURY REPORT FORM (F-OHS1)
To be filled in by the senior officer or NCO on the site of an incident involving injury to a Corps member, and where this is to be handed to the Health & Safety Officer within 24 hrs of the incident. The Health and Safety Officer should likewise be notified in the event of any “near misses” or similar incidents which occur and where persons could have but did not, sustain injuries.
